Specialty Care
Specialty care refers to a physician who has expertise in a specific field of medicine and can diagnose, treat and manage conditions related to their field of expertise. At Optum, our fellowship trained and board-certified Specialist physicians work closely with your primary care physician to help manage your care.

Acute Care
Having started as a Hospitalist services provider, 4C Medical Group, now Optum, has over 17 years of experience in providing high quality internal medicine services in Acute-Care settings.
We provide care to patients at several local acute-care hospitals and make it a priority to communicate with the patient’s primary care doctor whenever their patient is admitted under our Hospitalist team’s care.



Post-Acute Care
At Optum, we believe in being able to provide continuity of care when patients are admitted to Skilled Nursing and Acute Rehabilitation facilities after a hospitalization.
Our Post-Acute care physicians provide care to patients at over forty facilities in the Phoenix metro area and collaborate actively with care management teams to improve care outcomes and patient experience. We also communicate actively with the patient’s primary care physician so that they stay informed about the patient’s status and can effectively take over the patient’s care after discharge.

Medicare Advantage
Medicare Advantage is a different way to get Medicare. It covers everything that Original Medicare does, plus more. Medicare Advantage plans support your primary care doctor’s care plan. They help ensure you get preventative care like wellness visits, tests, checkups, and vaccines.



Virtual Appointments
